Biometric Screening - Wellness Platform

Know your numbers! For members covered under the Ascend to Wholeness Healthcare Plan, schedule a free biometric screening through the wellness platform on the Benefits page. You have several options for a biometric screening, including:
  • Primary Care Physician Form (take to your Preventive Care Visit)
  • Home Test Kit
  • CVS Minute Clinic Voucher
  • LabCorp Voucher
