Accidental Death and Dismemberment
- Active employees of the North American Division working a minimum of 19 hours a week are eligible.
- Employees may purchase coverage in the following amounts as requested by employee and approved by VOYA.
- $10,000 to $500,000 for the employee
- 100% or less of the employee amount for spouse
- $10,000 to $25,000 each for dependent children
- Follow instructions on claim form for completing form and filing claim.
- Follow instructions on the enrollment form to enroll a new hire or current employee.
- When employment ends, either due to retirement or leaving the organization, Accidental Death and Dismemberment terms as of the last day of the month of exit. There isn’t a provision for continuation of the policy.

Can I get coverage if I have a health problem?

Yes. There is no medical underwriting or medical exam required to purchase AD&D insurance.

Can this coverage be Ported or Converted when an employee leaves employment or retires?

No, the policy doesn’t have a provision for porting or converting.

Do I need AD&D insurance?

While you may have life insurance, AD&D insurance provides extra benefits such as loss of a limb or loss of sight. AD&D is a cost-effective way to cover expenses resulting from accidents.

Do I send anything to VOYA?

No, once you’ve entered the election amount in the VOYA spreadsheet, the enrollment form should be filed in-house

When can an employee enroll?

An employee may enroll at any time. Since there isn’t any medical underwriting, employees do not have to elect the coverage within 31 days of hire.
