Long-Term Disability

  • All employees in active employment with the employer in the United States or on temporary assignment for up to 1 year in Guam. You must be an employee of the employer and in an eligible class. Temporary and seasonal workers are excluded from coverage. Minimum hours requirement at least 20 hours per week or more, determined by your employer.
  • After a 90-day elimination period income is replaced at 66.67 percent to a maximum of $6,000 per month. Refer to your Policy Booklet for specific elimination period and income replacement percentage.
  • Claim Filing Instructions
    • Have the employee complete the Employee’s Statement in full, and read, sign, and date the Authorization for Release of Information.
    • Have the attending physician complete the Attending Physician’s Statement.
    • Complete the Employer’s Statement.

    • Do the other benefits provided by the North American Division continue once the employee is on LTD?
    • Yes, they do.
      • Enrollment in the health care plan would continue for 24 months starting on the first day of their elimination period.
      • Enrollment in the Group Basic Life would be continued for 18 months starting on the first day of their elimination period.
      • Contributions to their Retirement plan would be continued for 18 months starting on the first day of their elimination period.
    • Should the employee complete an FMLA form?
    • Yes, there is an elimination period of 90 days that must be met before LTD benefits are paid. This 90 days would run concurrently with the 12 weeks of Family Medical Leave Act.
    • When is an employee considered disabled?
    • An employee is considered disabled on the date that they meet the definition of disability as outlined in the VOYA LTD policy. An employee’s physician(s) information will help determine this date.
    • When should the claim be filed?
    • Disability claims should be submitted as soon as possible to ensure a claim decision is made in a timely manner and prior to the 90-day elimination period.
    • Who provides the employee on claim their W-2?
    • VOYA will produce and mail the W-2 to the individual while on claim.
